Description





Position Summary:

Provide Personalized Recovery Oriented Services to program participants, including individual and group counseling as well as case management services. Conducts assessments, develops Recovery Plans and maintains documentation of clients' participation and progress.


Essential Accountabilities:



+ Complete comprehensive assessments of individuals to determine their needs and appropriateness for enrollment into the PROS Program. Provide a written report of findings of assessments and develop recovery plans. This assessment will be used to document a patient's eligibility for services at the Program.


+ Write and maintain current treatment plan for each patient receiving services.


+ Document treatment in session notes that document the service provision and include as assessment of progress and review of the treatment plan.


+ Acts as leader/facilitator of activities for which program participants are assigned. Develops curricula and group protocols as necessary.


+ Participates in individual, group, family and other collateral meetings on and off-site.


+ Provides referrals to other programs and services.


+ Participates in the discharge planning process including initiation of all necessary referrals.


+ Acts as liaison with other agencies to provide information about PROS program and to facilitate new referrals.


+ Attends all scheduled staff meetings, and mandatory in-service training. Participates actively in individual and group supervision.


+ Communicates information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices.


+ Maintains required written documentation including written assessments, individual service plans and reviews, progress notes and contact notes.


+ Must be able to communicate information verbally and in writing for the purpose of giving instructions and providing feedback.


+ Work to achieve productivity standard set forth by administration.


+ Supervises student interns under direction of Program Coordinator/Clinic Director.


+ Responsible for participating in evacuations and ensuring program participants' safety in emergency situations (i.e. fire, fire drills, etc.).


+ Provides coverage as assigned for absent staff.


+ Comply with all Program, WellLife Network and OMH policies and procedures, including those regarding patient rights, documentation and incident reporting.


+ Able to lift/carry packages and boxes of supplies


+ Maintains caseload and insures medical record meets all agency and OMH requirements.


+ Responsible for reporting and documenting all incidents to immediate supervisor within 24 hours.


+ Represents program at meetings, trainings not otherwise specified.


+ Drive facility vehicle when the need arises.


+ Maintain confidentiality at all times.

Qualifications






- Bachelor of Art or Science degree with 3-5 years of behavioral health experience.


- Knowledge using electronic health records.
